Al-Razi (Rhazes): The Father of Paediatrics and Clinical Medicine
Al-Razi was the most clinically rigorous physician of the medieval world. He ran the first documented controlled experiment in medical history, differentiated smallpox from measles for the first time, and pioneered the application of psychology to medicine. His work shaped European medicine for centuries under his Latinised name, Rhazes.
Who He Was
Abu Bakr Muhammad ibn Zakariya al-Razi (854–925 CE), known in the West as Rhazes, was a Persian physician, philosopher, and alchemist born in Ray, near present-day Tehran. He is considered the greatest clinician in Arabic medicine and the father of paediatric medicine.
What He Did
The first controlled clinical trial: Al-Razi ran a documented medical experiment in which he divided meningitis patients into two groups (one treated with bloodletting and one not) and compared outcomes. This is the earliest documented use of a control group in medical history. The systematic testing of medical interventions that underpins modern evidence-based medicine begins here.
Smallpox and measles: Al-Razi wrote al-Judari wa al-Hasba, the first clinical description distinguishing smallpox from measles. The two diseases had been conflated throughout ancient medicine. His differential diagnosis was based on careful clinical observation and remained the standard reference on the subject in both the Islamic world and Europe for centuries.
Paediatrics: His Kitab al-Hawi fi al-Tibb (compiled by his students from his notes and published after his death) was one of the most comprehensive medical encyclopaedias of the medieval period. It contained detailed clinical descriptions of childhood diseases and established him as the father of paediatric medicine.
Psychology in medicine: Al-Razi was the first to propose that mental processes were localised in different parts of the brain. He described neurological conditions including epilepsy, migraines, and concussions in clinical detail that was centuries ahead of his contemporaries.
Hospital design: Al-Razi is credited with choosing the site for a new hospital in Baghdad by hanging pieces of meat at different locations across the city and selecting the site where the meat decomposed most slowly, an early application of environmental observation to public health.
What Was Attributed to Others
Al-Razi's Kitab al-Hawi was translated into Latin as Liber Continens and became a standard text in European universities. His clinical methodology (observing, recording, comparing outcomes) was the most rigorous in the medieval world. When European medicine began developing its own clinical tradition in the 17th and 18th centuries, the Islamic clinical framework on which it had rested was progressively obscured.
The controlled experiment (now considered the gold standard of medical research) was first documented by a Muslim physician working in 9th-century Baghdad.
A Note on Al-Razi's Views
Al-Razi held heterodox philosophical views and wrote polemical works against the concept of prophecy, views rejected by mainstream Islamic scholarship. We know about his heterodox positions primarily from hostile sources, who may have mischaracterised them. His medical contributions stand independently of his theology and represent one of Islamic civilisation's most significant gifts to medicine.
